The ordering provider must determine the best INR level depending on the reason for anticoagulant treatment and how each individual responds to treatment. For patients taking warfarin, an INR of 2.0 to 3.0 is generally considered an effective therapeutic range.
INR | Required Action |
Establishing new patients | Send to CPH for confirmatory testing |
>/= 4.0 | Send to CPH for confirmatory testing |
>/= 6.0 | Repeat POC test and send to CPH for confirmatory testing |
> 1.0 change since last visit | Repeat POC test and send to CPH for confirmatory testing |
>/= 0.5 difference between two consecutive tests. (same visit) | Send to CPH for confirmatory testing |
Any unexplained clinically significant change Significant change, but now in therapeutic range <0.8 |
